

There was a large-scale outage of websites including iCloud, Slack, Quora and even the US Securities and Exchange Commission because an Amazon employee entered a wrong command while removing servers from an S3 subsystem. Some servers were to be removed and by mistake he entered a larger number than intended. By mistake, two among these servers closed down by mistake manage the storage and metadata for the entire region that was shut down. More details on this link.
